Champlain Cardiovascular Disease Prevention Network (CCPN)
The CCPN is a group of health and community partners from across the Champlain District of Ontario that is providing leadership to the implementation of a five-year Cardiovascular Disease (CVD) Prevention Strategy. The Strategy seeks to reduce the burden of heart disease and stroke and ensure the residents of the Champlain District are the most heart healthy and stroke-free in Canada. |

The mission of the Eastern Ontario Health Unit (EOHU) is to promote and protect the health and well-being of its citizens by preventing diseases and accidents, and advocating for a safe and healthy environment and healthy public policies. Ottawa Public Health
Ottawa Public Health (OPH) is responsible for delivery of public health services including health protection, disease and injury prevention, control of communicable diseases, and family health services. OPH offers bilingual programs and services to Ottawa residents in both rural and urban communities, spanning Cumberland in the east to Fitzroy Harbour in the west, and from the Ottawa River in the north to Osgoode in the south. |

Renfrew County and District Health Unit
With its mandate of “promoting healthy people in a healthy environment”, the Renfrew County & District Health Unit (RCDHU) covers a large geographic area that includes the County of Renfrew, the City of Pembroke, the Township of South Algonquin in the District of Nipissing and most of Algonquin Provincial Park. The RCDHU’s main office is located in Pembroke, with branch offices in Arnprior, Barry’s Bay, Deep River and Renfrew. |

Diabetes Network members and the Champlain Local Health Integration Network (LHIN) work collaboratively to promote the development of a system of comprehensive, accessible, culturally and linguistically appropriate, evidence-based diabetes services, including advocacy, prevention, clinical care and self management education for all individuals in the Champlain region who are at risk for, or are living with diabetes.
